Context: There is positive correlation between infant serum ghrelin and the growth of infants in breastfed infants.
Objective: The purpose of this study was to evaluate association of breast milk ghrelin and serum ghrelin and growth in breast milk fed infants by Systematic Review and Meta-analysis.
Data sources: Electronic resources in the search were Scopus, Cochrane Library, Science, PubMed and EMBASE. The articles in English and articles with English abstract were collected from 1999 until July 2019.
Study selection: Only the articles were reviewed that contained infants under two years of age and receiving breast milk exclusively. Papers related to healthy mother and the infants entered in this study. The results of six papers were related to the effect of serum ghrelin on growth of infants that were evaluated by meta-analyzing using Stata software. The results of the effect of breast milk ghrelin on growth are reported by systematic review. Three articles were found in this area.
Data Extraction: We used Preferred Reporting Items for systematic reviews and meta-analyses (PRISMA) checklist for carrying out this study.
Results: In this study, the association of infants’ serum ghrelin with infants’ growth was evaluated by meta-analysis. Newcastle-Ottawa Scale (NOS) was equal and higher than seven scores for six meta-analysis articles. The present meta-analysis showed a positive and significant correlation between infant serum ghrelin and the growth of infants (r = 0.163). 
Conclusion: The results of random-effect model demonstrated a positive and significant correlation between infant serum ghrelin and the growth of infants. On the other hand, in the study of the effect of breast milk ghrelin on infants’ growth, the number of articles was limited and contradictory results were reported. In this case, no general conclusion was reached.
